Family & Internal Medicine Behavioral Health Services Offered Assess for and diagnose mental health disorders in ages 4-65 Provide medication management services for pateints with mental health issues Help patients understand diagnosis and treatment option Discuss non-pharmalogical treatment options for mental illness Provide brief therapy for patients using motivational interviewing and cognitve behavior therapy Provide psychopharmacology education Referral Patients need to first discuss needs with a primary care provider from MHP Family & Internal Medicine, MHP OB/GYN, or MHP Pediatrics. Counseling and Intervention for Family and Internal Medicine Patients: Provide brief therapy treatment to patients 15 years of age and older Treatment for anxiety/panic, depression, anger, and trauma Assist patients in coping with illness and grieving loss Provide psychoeducation on mental health diagnosis Address and change unhelpful thought distortions Development of healthy coping strategies Improvement of self-esteem Development of healthy lifestyle Support for LGBTQI+ populations Assist with social determinates of health Referral Patients can be referred by a primary care provider, psychiatrist, psychiatric nurse practitioners, or a chronic case management nurse. Patients reporting and/or exhibiting distress are referred to the social worker for social services and support.
