Why Sports Physicals Are Important

Getting a sports physical is an important step before joining a school or team sport. A sports physical helps make sure your child is healthy and ready to play sports safely. During the exam, a medical provider checks for health problems that could cause injuries or affect sports performance.

What to Bring to Your Sports Physical

To help your visit go smoothly, please bring:

  • A copy of the IHSAA Sports Physical Form for us to review and sign.

  • Prescription glasses or contact lenses if your child wears them.
