Nominating them for a Major Angel by making a gift to the MHP Foundation is a meaningful way to express your appreciation. Your angel will be presented with a special pin in front of their peers, and if you would like to send them a special message, the form below includes a box to do so.

Memorialize a loved one with a gift to the MHP Foundation

Memorial gifts help make “the MHP difference” for our patients and the community, so they are a very worthwhile tribute! The family of the person memorialized will be notified of your gift, but the amount will not be disclosed.
