MHP Diabetes Clinic

The MHP Diabetes Clinic helps people manage diabetes and improve their health. Our team includes Registered Nurses, Registered Dietitians, and Certified Medical Assistants who work with your primary care provider or endocrinologist to improve outcomes and reduce risks.

We accept referrals from providers inside and outside of Major Health Partners and provide care in a supportive, judgment-free environment for patients of all backgrounds.

We Help Patients With:

  • Type 1 Diabetes
  • Type 2 Diabetes
  • Gestational Diabetes (during pregnancy)
  • LADA (Latent Autoimmune Diabetes in Adults, also referred to as “Type 1.5)
  • MODY (Maturity-Onset Diabetes in the Young)

What to Expect

After you are referred to our clinic, our team will call you to schedule your first visit. Your first appointment with a diabetes nurse may last up to one hour. Together, we will create a care plan that fits your needs and goals.

Telehealth visits may be available when appropriate.

Diabetes Education and Support

Our Diabetes Care and Education Specialists can help you with:

  • Healthy eating
  • Physical activity
  • Blood sugar monitoring
  • Medications and insulin
  • Reducing health risks
  • Problem-solving skills
  • Healthy coping strategies

Diabetes Technology

We help patients use the latest diabetes technology, including:

  • Continuous Glucose Monitors (CGMs)
  • Insulin pumps

Language and Accessibility Services

MHP offers interpreter services for non-English-speaking patients and ASL support for hearing-impaired patients. Educational materials can also be adapted for patients with low vision, low literacy, or other special needs.

Accredited Diabetes Program

The MHP Diabetes Clinic follows the latest American Diabetes Association Standards of Care and is accredited by the Association of Diabetes Care and Education Specialists (ADCES).
