Future of Medicine Program

Through MHP’s partnership with Care Access: Future of Medicine, eligible adults may receive specialized health screenings that can help identify important risk factors and provide valuable information to share with their healthcare provider. Based on the results of the labs, the patient may be invited to participate in current research studies.

Do I need my physician to refer me to this program?

No, a physician referral is not required. The physician may recommend the patient to have the labs done, but it is not mandatory to participate in this program.

What is the cost of this program?

This program is completely free for the patient to participate in, as long as they meet the eligibility criteria.

What is the eligibility criteria?

Participants must be at least 18 years old and have at least ONE of the conditions below:

  • BMI 25+
  • Heart attack
  • Stroke
  • Stent
  • Open heart surgery
  • CHF
  • Diabetes
  • Pre-diabetes
  • CKD
  • High blood pressure
  • High cholesterol
  • Current smoker
  • Sleep Apnea
  • Male age 65+
  • Asthma
  • COPD
  • Crohn's Disease
  • Ulcerative Colitis
  • IBS
  • Liver disease
  • Osteoarthritis
  • Other Chronic Pain
  • Family history of Diabetes
  • Family history of Alzheimer's
  • Poor circulation
  • CAD / PAD
  • Osteoarthritis (knee)
  • Female age 70+

*NOT eligible if diagnosed with Alzheimer's disease and/or currently participating in a clinical trial

When are the walk-in hours?

There is a 4 hr block of time scheduled each week where patients can come in to get their blood drawn.
