Our approach focuses on building lasting relationships, managing both immediate and long-term health needs, and supporting your overall well-being where you live. Your care team serves as your primary point of contact for in-home medical care, helping reduce unnecessary hospital visits while ensuring your care remains personalized, proactive, and connected. We work closely with you to understand your goals, coordinate services, and support your health every step of the way.

What to Expect:

• Initial Wellness Exam, followed up by Annual Wellness Exams
• Recommended screenings
• Medication reviews, vaccines
• Discussions about lifestyle, safety, and chronic disease management

Additional Resources Available:

• Assistance with Advance Directives
• Referrals to additional services such as specialists, therapists, diagnostic testing, home health agencies, etc.
