Diabetes Education at MHP

 

The MHP Diabetes Clinic offers specialized Diabetes Education classes where you can learn everything you need to know about the day-to-day aspects of diabetes. Whether you have just been diagnosed with diabetes, have had diabetes for years or you have Gestational Diabetes, we have the resources to help you manage and treat your diabetes.


 

Have you recently been diagnosed with diabetes? Start with our Diabetes 101 class and learn everything you need to know.


MHP's Diabetes 101 class is led by specially trained diabetes clinicians, our Comprehensive Diabetes Self-management Training Program is designed to help you understand the disease and manage its symptoms, as well as reduce your risks for complications. This class is recommended for all patients that are newly diagnosed or that have never received formal diabetes education.
The program is held in the MHP Health Pavilion (Entrance 3). It includes three 2 hour sessions, where you can learn more about:
 

  • Diabetes disease process and management
  • Meal planning and carbohydrate counting
  • Healthy strategies for dining out and grocery shopping
  • Activity and exercise planning
  • Glucose monitoring and control
  • Medication information and instruction
  • Sick day management
  • Proper foot care
  • Coping with lifestyle changes
  • Setting healthy goals and staying motivated


Our Comprehensive Diabetes Self-management Training Program meets National Standards for Self-management Education. These standards are design to define quality and ensure that the program is effective in providing evidence-based instruction and support needed by diabetes patients to manage the disease successfully. The standards also make certain that our program is comprehensive and designed to meet the individual needs of all participants.

Do you have diabetes and would like to improve your blood sugar management? Begin here with our Diabetes Refresher class.
 

MHP's Diabetes Refresher class is led by specially trained diabetes clinicians, our Comprehensive Diabetes Self-management Training Program is designed to help you understand the disease and manage its symptoms, as well as reduce your risks for complications. This class is recommended for anyone with diabetes that is interested in improving their blood sugars or A1C by reviewing key diabetes management strategies and concepts.
The program is held in the MHP Health Pavilion (Entrance 3). It includes one 2 hour session, where you can learn more about:
 

  • Diabetes disease process and management
  • Meal planning and carbohydrate counting review
  • Healthy strategies for dining out and grocery shopping
  • Activity and exercise review
  • Glucose monitoring and control
  • Medication management
  • Setting healthy goals and staying motivated

MHP's Gestational Diabetes Education is led by specially trained diabetes clinicians, our Comprehensive Diabetes Self-management Training Program is designed to help you understand the disease and manage its symptoms, as well as reduce your risks for complications. This education program is recommended for all women who have developed diabetes during their pregnancy.
The program is held in the MHP Health Pavilion (Entrance 3). It includes three 1 hour sessions, where you can learn more about:

 
  • Diabetes disease process and management
  • Meal planning and carbohydrate counting
  • Healthy strategies for dining out and grocery shopping
  • Activity and exercise
  • Glucose monitoring and control
  • Medication management and instruction
  • Sick day management
  • Setting healthy goals and staying motivated
