Diabetes Clinic

The MHP Diabetes Clinic offers inpatient and outpatient education for diabetic patients and their families. Our nurses and dietitians have special training in diabetes care and education.

Education and management of the following are available at MHP:

• Type 1 and Type 2 diabetes
• Gestational Diabetes (GDM)
• Insulin pump education and placement
• Continuous Glucose monitoring (personal and professional)
• Medical Nutrition Therapy

We will also work closely with your primary care provider to assist in the management of diabetic medications, insulin and insulin pump settings.

Major Health Partners follows the ADA National Standards for Diabetes Self-Management Education (DSME). Patients are encouraged to take an active role in managing their diabetes. Through our education program, you can gain confidence, essential knowledge and skills to manage your diabetes, reduce risks of complications and improve quality of life.
